KOWALCZYK John "Jack" Kowalczyk, age 60, of Worthington, died Thursday September 21, 2000 at Riverside Methodist Hospital. Jack was Claim Counsel for State Auto for 30 years. He is preceded in death by his parents John and Ann (Manka) Kowalczyk. He is survived by his wife of 32 years, Donna Kowalczyk; daughters, Christa Kowalczyk, of Tarui-cho, Japan and Alisa (Keith) Agin, of Columbus and many other family members and friends. He was a member of the American Bar Association, Columbus Power Squadron and All Saints Lutheran Church. A graduate of Roanoke College in 1963, Jack also graduated from the University of Tennessee Law School in 1967. Family will receive friends from 5-8 p.m. Monday at RUTHERFORD-CORBIN FUNERAL HOME, WORTHINGTON CHAPEL, 515 High St., Worthington, Oh.
