Collins Phyllis J. Collins, age 83, of Worthington, passed away peacefully, Friday, October 18, 2013 at home. Preceded in death by parents Val and Ernestine Cook and brother James E. Cook. Survived by her husband, Charles Collins; children, Terry (Paul) Moore, Krista (Rick) Hawk, Charles (Cathy) Collins III, Jeff Collins and Johnathon (Trixie) Collins; grandchildren, Dean Moore, Ray (Sarah) Moore, Megan and Karianne Stalnaker, Jennifer (Colin) Shea, Andrew and Kimberly Collins, Jessica and Alan Collins, Jeffrey McCauslin, Philip and Aaron Collins; great-grandchildren, Kayla and Cohen Shea. 1948 graduate of Lebanon H.S., Lebanon, Ohio. Member North Baptist Church. Funeral Service will be held 11:30 a.m. Tuesday, October 22, 2013 at RUTHERFORD-CORBIN FUNERAL HOME, 515 High St., Worthington, Ohio 43085. Rev. Cyrus Broughton officiating. Family will receive friends 3-7 p.m. Monday. Burial Walnut Grove Cemetery. In lieu of flowers, contributions may be made to the North Baptist Church.
